Community Health Choice is a local, nonprofit, managed-care organization committed to helping improve the health and well-being of Texas residents. We serve over 475,000 Members. We’ve served Houston and surrounding counties as a trusted partner since 1997, and we are proud to offer the largest medical network in Southeast Texas. In 2020, we began offering a Medicare Advantage Dual-Eligible Special Needs Plan (HMO D-SNP) for people who are eligible for both Medicare and Medicaid.

Community Health Choice HMO D-SNP offers Members extra benefits and features, like dental coverage and a quarterly allowance for rent and groceries, along with friendly service and a large network of doctors and healthcare facilities. Community Health Choice (HMO D-SNP) also helps make your Medicare and Texas Medicaid benefits work together and work better for you.

Being able to work with one health plan for all your health insurance needs.
Having a care management team that may include primary care physicians, nurses, counselors, and other health professionals who are there to help you get the care you need.
Having a care coordinator who works with you and your care management team to help guide your healthcare decisions.
