How to Enroll in our (HMO D-SNP)

Step 1: Check your eligibility.

Step 2: Learn about our plans.

Call Member Services for detailed information about our D-SNP plans.
  • Call 713.295.5007 (local) or 1.833.276.8306 (toll-free). TTY users should call 711.
  • October 1 to March 31, 8:00 a.m. to 8:00 p.m., 7 days a week, and April 1 through September 30, 8:00 a.m. to 8:00 p.m., Monday through Friday.
  • On certain holidays your call will be handled by our automated phone system.

Step 3: Complete an application.

We have three ways for you to apply – by phone, mail or online. Choose the way that works best for you!

Step 5: Confirm your enrollment.

  • If you are found eligible for the Community Health Choice (HMO D-SNP) program, you will receive your enrollment confirmation in the mail.

When You Can Enroll in a D-SNP

Enrolling in a D-SNP is easy, but when you can enroll depends on two things:

  1. If you are enrolling in a D-SNP for the first time: You can enroll at any time of the year with no restrictions.
  2. If you already have both Medicare and Medicaid, or you are already enrolled in a D-SNP: You can switch plans once during a Special Enrollment Period (SEP) or during the Medicare Annual Enrollment Period (AEP).

Out-of-Network Coverage Rules

If Covered Services are not available through in-network Participating Physicians or Providers, you or your doctor may request prior authorization for Covered Services by a doctor or other provider outside the contracted network. If Covered Services are approved as described above, then you are only responsible for your share of the cost, if any, that you may have paid if you could obtain services from Community’s network of Participating Providers. If you receive a bill from an out-of-network provider, contact Member Services for assistance.


Community Health Choice Texas, Inc. is a (HMO D-SNP) plan with a Medicare contract. Enrollment in our plans depends on contract renewal.
