Community Health Choice (HMO D-SNP) is a special type of Medicare Advantage plan. This special plan, also known as a Dual Eligible Special Needs Plan (D-SNP), is for individuals who qualify for both Medicare and Medicaid. Some of the advantages include:

HOW DOES COMMUNITY HEALTH CHOICE (HMO-D-SNP) WORK?

Unlike Original Medicare, with Community Health Choice (HMO D-SNP), all of your coverage – medical, hospital, prescription drug coverage – is included. Instead of relying on Medicare Part A and Part B to cover hospital and medical expenses, and a separate Part D plan for prescription drugs, everything is conveniently covered by one simple, comprehensive plan.
