FLEX CARD BENEFITS BENEFITS1

Our plan makes it easy for you to get the additional help you need for necessary items like groceries, rent and utility assistance.

Allowance varies based on the county in which you reside. Please check the summary of benefits. Quarterly allowance cannot be carried over from one quarter to the next. The benefits mentioned are a part of special supplemental program for the chronically ill. Not all Members qualify.

OVER-THE-COUNTER (OTC) BENEFITS1

As part of your plan, you can save on eligible OTC medicines and health-related items by using your OTC benefit.

Save money Receive up to $220 in OTC drugs and supplies every quarter. That’s a savings of at least $880 over the course of a year!

Save time – The pre-loaded OTC card offers a convenient way to get OTC health and wellness products. No need to wait for your items to be delivered in the mail. You can purchase your needed items in real-time and take them home with you.

OTC HEALTH AND WELLNESS PRODUCTS AVAILABLE TO PURCHASE INCLUDE:

  • Pain relief medications
  • First-aid supplies
  • Aspirin
  • Dental supplies
  • Vitamins and supplements
  • Cough, cold, and allergy medicine
  • Incontinence products
  • Leg and foot care products
  • Mobility support
  • Bath and safety products
  • Digestion/laxatives/antacids
  • Medication management items
  • Ear and eye care
  • Miscellaneous items, such as cleaning
    wipes, cotton swabs, and lotions

Note: Please check with your doctor or medical Provider before taking any over-the-counter medication.

PAPA PALS BENEFITS1

Papa connects college students to older adults who need transportation, house help, technology lessons, companionship, and other support services.

Papa Pals are young, fun, energized, and ready to provide unique experiences to older adults. Think of them as “on-demand grandkids” who can make aging fun and support older adults through this part of life’s journey.

They help with all sorts of things, such as running errands, grocery shopping, doing laundry, light cleaning, and meal preparation.

Papa Pals are trusted, friendly, and carefully vetted to ensure the greatest experience and peace of mind for you and your loved ones.

Companionship Services – Papa Pals provide amazing companionship to Members, taking them around town, hanging out, watching movies, playing board games or having great conversations.

Housekeeping – Papa Pals can help you or your loved ones with everyday household tasks, including laundry, light cleaning, meal preparation, and taking care of pets.

Social Transportation – Papa Pals provides older adults with transportation and companionship to and from their destinations, including the doctor’s office, the airport, community centers, jobs, and other locations. Four hours a month are provided at $0 cost to Members. Additional hours may be purchased by the Member.

Grocery Shopping – Papa Pals assist with grocery shopping and delivery by picking Members up at their homes, taking them to the supermarket, carrying their shopping bags, and taking them back home.

Caregiver Support – Community Health Choice understands caring for a loved one is not always easy. This is why our plan provides caregiver support services through:

  • In-home support services
  • Performing errands
  • Clothes shopping
  • Meal preparation and delivery
  • Respite care
  • Social interactions through religious and
    spiritual connections
  • Community engagements
  • Rx delivery
  • Group experiences such as art therapy, pet therapy, cooking, and music therapy 

Technology Lessons – Papa Pals can help Members with technology by teaching them how to use a computer, smartphone or tablet; how to use social media; and how to video chat with grandkids and other loved ones.

Community Health Choice offers its PYX Health to its (HMO DSNP) members. Get the care you need now.

Because everyone could use a little extra support, Pyx Health helps you find the right care when you need it. Our one-of-a-kind combination of compassionate humans and simple mobile app work together to make self-care easier.

With Pyx Health’s science-based tools and resources, Members can:

  • Stay connected to all health plan benefits
  • Talk with caring staff for support
  • Find doctors, support groups, food, transportation, diaper services, and more
  • Get resources to improve mood, anxiety, motivation, and more

To ensure our Members know how to use their benefits, our plan provides training on how to use the Pyx Health mobile app.

TRANSPORTATION SERVICES

Community Health Choice understands Members may need assistance getting to the doctor or other medical appointments. This is why our plan provides one-way transportation services for Medically Necessary services. Covered transportation may include car services such as Uber, ambulance rides or other means of medical transport, depending on your medical needs. Members can get up to 4 one-way trips per month to plan-approved medical locations at $0 cost. Plan approved locations include:
  • Doctor’s offices
  • Outpatient facilities / centers
  • Clinics
  • Other healthcare-related locations
Service Details:
  • No mileage radius
  • Allows one caregiver or translator to accompany Member on rides
  • 3-day advance notice is preferred but not required
  • An additional stop is allowed but Members must call (insert the number here?) and notify Community in advance (will count as a trip)
Transportation must be approved and provided by Community Health Choice’s contracted transportation vendor.

TELADOC

Community Health Choice offers 24/7 virtual care services* to all (HMO DSNP) Members through our provider, Teledoc 

To use Teledoc, set up your account on Teladoc.com or the Teladoc mobile app before your first visit. You can also call 1-800-Teladoc. 

WAYS TO USE VIRTUAL CARE:

  • Talk to a doctor to treat routine medical concerns 24/7 from anywhere by phone, web or mobile app
  • Get a new prescription or a refill
  • To ensure our Members know how to use their benefits, our plan provides training on how to use virtual primary care services.

  • Your primary care doctor is not available

  • If you are thinking about visiting the ER or Urgent Care for routine medical concerns

DENTAL BENEFITS
PROVIDED

Did you know that getting regular dental visits can help prevent medical problems such as heart disease? The connection between your dental health and physical health begins at your mouth. Dental problems like tooth fractures and gum disease can indicate underlying health problems. Other health problems can even be caused by your dental health. At Community, we care about your overall well-being, which is why Community Health Choice (HMO D-SNP) includes additional dental coverage. Plan includes two routine dental visits each calendar year that cover:
  • Bitewing X-rays (set of two)
  • Cleaning
  • Oral exams
Additionally, we provide a $3,500 annual allowance for comprehensive dental services that covers:
  • Palliative emergency treatment
  • Periapical X-rays
  • Amalgam and composite fillings
  • Simple (non-surgical) extractions6 (prior approval may be required for oral surgeon services)
  • Emergency exams
  • Major restorative (inlays and onlays)
  • Dentures
  • Adjustments and repairs of prosthetics
  • Coverage for dentures
6Coverage for simple extractions (non-surgical extractions) are typically performed by a general dentist. Surgical extractions are typically performed by an oral surgeon are covered by our plan but may require prior approval.

Our hearing changes as we age, which can have a profound impact on our quality of life. It’s important to protect your hearing and even more important to know when it is time to seek help for hearing loss. 

With Community Health Choice (HMO D-SNP), diagnostic hearing and balance evaluations to determine if you need medical treatment are covered at a $0 copay as outpatient care when furnished by a physician, audiologist or other qualified Provider.   

You receive: 

  • One routine hearing exam every year
  • Benefit allowance of $1,000 for hearing aids, one-per ear, every two years.

Program features:

  • $0 hearing aid fitting and evaluations
  • Annual hearing test with no out-of-pocket cost
  • Access to a large network of trusted Providers
  • Brand-name hearing aids available from all major manufacturers

VISION BENEFITS

The Community Health Choice (HMO D-SNP) provides more vision benefits than Medicare Part B to help our Members have the healthiest vision possible.

Many eye conditions — including ones that cause blindness — don’t have early symptoms. And some serious diseases that affect other systems in the body can first be diagnosed during a yearly complete eye examination.

Our services include:*

  • Routine eye examination once every calendar year
  • $0 cost for standard prescription eyeglass lenses
  • $350 annual plan allowance for eyeglass frames or contact lenses once every two calendar years

*Payment will be made for either frames or contact lenses within a benefit period. Payment will not be made for both. Services and allowances are based on a 12-month enrollment.

Extra options for lenses – such as but not limited to polycarbonate, transitions, non-line progressive bifocals, and trifocals – are not covered.

At Community Health Choice, we recognize that our Members have individual needs that require unique solutions, so we assign a Care Coordinator to act as a central point of contact for your healthcare needs.  

Your Care Coordinator will:  

  • Create an Individualized Care Plan (ICP) to help understand your current and future healthcare needs 
  • Assist with scheduling medical appointments, discuss rehabilitation options, and access durable medication equipment 
  • Connect you to community resources like transportation, food assistance, financial aid, and behavioral health services 
  • Ensure that you are making the most of your Community Health Choice (HMO D-SNP)
    benefits

POST-HOSPITALIZATION MEALS

Eligible Members who are discharged from a hospital receive 14 meals (2 meals per day for 7 days) as part of their benefit plan at no additional cost. The fully prepared and nutritionally balanced meals are designed to enhance recovery and healing. 

Members can choose from the following specialty diet plans that address major health concerns: 

  • Diabetic
  • Heart-healthy
  • Renal friendly
  • Lower sodium
  • Gluten free
  • Vegetarian
  • Pureed

PART D PRESCRIPTION DRUG COVERAGE
is included

With the Community Health Choice (HMO D-SNP), Members have the peace of mind that comes from not having to purchase a separate Medicare drug plan. With our plan, your Part D prescription drug coverage is included, and it does not have a deductible to satisfy before your benefits begin.

SELF-DIRECTION1

SERVICES SUPPORTING SELF-DIRECTION

Our self-directed care program assists in managing all aspects of personal healthcare delivery. 

Services include counseling on how plan benefits work, and provide training and technology assistance for our Pyx Health mobile app companion service.

We also offer counseling services regarding community based programs that assist with home repair, applications for subsidies for utility bills, and other socio-economic needs.
