We offer a plan called Community Health Choice (HMO D-SNP), which has a $0 plan premium with $0 prescription drug copay along with many other benefits.

The Community Health Choice (HMO D-SNP) also offers:

• More than 16,000 providers and 1,300 pharmacies in our 20-county network
• More than $6,000* annual allowance 

WHEN YOU ENROLL IN COMMUNITY HEALTH CHOICE (HMO D-SNP), YOU CAN GET**:

  • Preventive dental care + $3,500 allowance for all covered dental services including dentures and bridges
  • Routine vision exam
  • 48 one-way trips to medical appointments
  • 2 meals per day for 7 days after a hospital stay
  • $350 allowance for glasses/lenses/ contacts
  • $220* quarterly over-the-counter (OTC) pharmacy allowance****
  • Routine hearing test and one hearing aid per ear per year ($1,000 max per ear every two years)
  • Acupuncture visits up to 24 visits per year
  • Additional chiropractic visits up to 24 visits per year
  • PCP Virtual Care Services
  • ***Emotional Support to assist with social isolation – Emotional support and companionship services through Pyx Health mobile app mobile application
